About the role
- The Partner Service Professional plays a vital role in supporting Elara Caring’s mission by managing the referral process and ensuring seamless communication across teams.
- This position is responsible for converting referrals into active services, entering and maintaining accurate data, answering inquiries, and sharing information about the agency.
- The Partner Service Professional also coordinates referrals, supports daily operations, and fosters effective communication between Clinical Supervisors and the agency to help secure and manage patient care needs.
- Stay engaged in a fast-paced, high-energy environment where you’ll handle about 60–65 calls each day.
- Help patients and families by setting up services quickly and seamlessly when new referrals come in.
- Work closely with our Clinical Supervisors and team members to keep communication flowing and patient care on track.
- Maintain accurate, confidential records while ensuring all referrals are properly documented and tracked.
- Provide excellent customer service to both patients and partner organizations.
Requirements
- Associate’s degree preferred
- Minimum of 1 year of experience in a healthcare setting (required)
- Strong knowledge of medical terminology
- Excellent communication and interpersonal skills
- Ability to complete daily tasks accurately and efficiently
- Comfort working with patients who may be ill, disabled, or at end-of-life
- Capacity to thrive in a fast-paced, high-stress environment
- Physical ability to sit, stand, bend, lift, and move intermittently
- Receiving call Center experience is highly preferred
- Will need to live in a State where there is an Elara Caring Branch
