About the role

Key responsibilities & impact
  • play a key role in the day-to-day management of the team
  • undertake Care Act assessments and reviews of adults and carers who have been referred to your team and who are ready for discharge from hospital
  • work in a multidisciplinary way to support a person’s discharge and contribute to discussions that promote the person’s independence and best interests
  • be responsible for oversight of the safeguarding enquiries held by the team
  • ensure good outcomes for the individuals we are working with by working with local Integrated Commissioning Boards (ICB’s)

Requirements

What you’ll need
  • Can work collectively with colleagues, both internally and externally
  • Have experience of supervising Adult Social Care professionals or the potential to do so, for example development work in a health or social care setting
  • Have the ability to make decisions when dealing with often constant and conflicting demands on time.
