About the role

Key responsibilities & impact

play a key role in the day-to-day management of the team
undertake Care Act assessments and reviews of adults and carers who have been referred to your team and who are ready for discharge from hospital
work in a multidisciplinary way to support a person’s discharge and contribute to discussions that promote the person’s independence and best interests
be responsible for oversight of the safeguarding enquiries held by the team
ensure good outcomes for the individuals we are working with by working with local Integrated Commissioning Boards (ICB’s)

Requirements

What you’ll need

Can work collectively with colleagues, both internally and externally
Have experience of supervising Adult Social Care professionals or the potential to do so, for example development work in a health or social care setting
Have the ability to make decisions when dealing with often constant and conflicting demands on time.
