About the role

Key responsibilities & impact
  • Provide specialist assessments under the Care Act and Mental Capacity Act to create strength-based support plans.
  • Using a trauma informed approach, develop safety planning and ongoing support to adults facing homelessness or domestic abuse.
  • Work closely with housing, health, police and voluntary sector partners, lead on safeguarding concerns, ensuring statutory duties are met.
  • Expected to work flexibly across community settings, including outreach, sourcing emergency accommodation, and working in partnership with agencies.
