About the role

Key responsibilities & impact

Provide specialist assessments under the Care Act and Mental Capacity Act to create strength-based support plans.
Using a trauma informed approach, develop safety planning and ongoing support to adults facing homelessness or domestic abuse.
Work closely with housing, health, police and voluntary sector partners, lead on safeguarding concerns, ensuring statutory duties are met.
Expected to work flexibly across community settings, including outreach, sourcing emergency accommodation, and working in partnership with agencies.

Can work collaboratively as a member of a team and be able to develop professional relationships across teams and services to achieve shared aims.

Understand and have the ability to undertake assessment and review activity applying good practice in relation to individuals and carers.
