About the role
Key responsibilities & impact- Collaborate with the multidisciplinary team to support people within their homes.
- Assess the persons current function/needs and create and implement a tailored treatment plan.
- Enable people to regain independence and continue to live safely at home.
- Help people get back to being independent following a period of illness or injury.
Requirements
What you’ll need- Has BSc in Physiotherapy/post grad MSc Physiotherapy with a HCPC registration
- Have the ability to organise, prioritise and delegate workload
- Are able to respond positively to change and to implement change initiatives
- Are motivated to work flexibly within an inter-professional framework
- Have the ability to meet the travel requirements of the role.
