Key responsibilities & impact

Overseeing the Hospital Community Discharge Teams and the Continuing Health Care Team
Managing community hospital inpatient assessments and discharges
Facilitating smooth transitions through community health pathways
Managing a team of Practice Managers
Leading and managing the ASC Continuing Health Care Team alongside two Community Hospital Discharge Teams
Ensuring all practice complies with statutory duties under the Care Act
Chairing complex multi-agency meetings
Working closely with the Service Management Team to oversee cases involving Court of Protection applications and Coroner’s Inquests

Requirements

What you’ll need

Translating local and national policy into practice
Contribute to and implement new service initiatives
Manage budgets and display effective leadership skill
Interpret legislation in difficult cases
Display working knowledge of the Capacity Act (2005) and DoLs
