About the role
- Develop, implement, and evaluate patient plans for medical care, therapy, and training services.
- Support clinical disease management and assess disease states and utilization.
- Collaborate with interdisciplinary teams to ensure excellence in patient care and optimal clinical outcomes.
- Provide individual counseling on rehabilitation treatment and health maintenance.
- Document interventions within medical records to collaborate with healthcare providers.
- Conduct community encounters with patients and care management team members.
- Participate in quality/performance improvement projects.
Requirements
- Bachelor's degree in business, behavioral/social sciences, public health or related population health field.
- 3 years of experience in a business, behavioral/social sciences, public health or related population health field.
- Sales and Marketing background.
- Professional experience in Social Work, Disease Management, and working with Physicians and Advanced Practice Providers is preferred.
