About the role

  • Develop, implement, and evaluate patient plans for medical care, therapy, and training services.
  • Support clinical disease management and assess disease states and utilization.
  • Collaborate with interdisciplinary teams to ensure excellence in patient care and optimal clinical outcomes.
  • Provide individual counseling on rehabilitation treatment and health maintenance.
  • Document interventions within medical records to collaborate with healthcare providers.
  • Conduct community encounters with patients and care management team members.
  • Participate in quality/performance improvement projects.

Requirements

  • Bachelor's degree in business, behavioral/social sciences, public health or related population health field.
  • 3 years of experience in a business, behavioral/social sciences, public health or related population health field.
  • Sales and Marketing background.
  • Professional experience in Social Work, Disease Management, and working with Physicians and Advanced Practice Providers is preferred.
