About the role

Key responsibilities & impact

Conduct focused and thorough assessments of patients with hypertension, hyperlipidemia, coronary artery disease, and diabetes who are running out of medications
Perform virtual visits, comprising an expected 40–60% of clinical effort
Formulate diagnoses and individualized treatment plans, including medication management, lifestyle modifications, and monitoring recommendations
Manage a prescription refill inbox and queue according to established protocols and guidelines
Participate in centralized results management, including review, triage, and follow-up on lab and diagnostic results
Use Digital live portal chat and AI-assisted clinical tools for prescribing, inbox triage, medication renewal, results management, and patient communication
Identify risk factors, comorbidities, and treatment contraindications
Counsel patients on medication adherence and potential side effects
Collaborate with PCPs, specialists, pharmacy, clinical nursing, social work, family members, and caregivers
Use the company’s electronic health information system for virtual visits and provide interface feedback
Maintain accurate patient medical records in compliance with legal and ethical guidelines
Participate in quality improvement initiatives and ongoing professional development
Adhere to laws, regulations, industry standards, patient privacy requirements, and telehealth regulations

Requirements

What you’ll need

Advanced Practice Provider (NP or PA)
Three or more years working in outpatient clinical practice
Minimum of 2 years concentrated in primary care or a relevant subspecialty with heavy focus on hypertension and lipid management
For Nurse Practitioners: active and in-good-standing RN and APRN licenses
For Physician Assistants: active and in-good-standing Physician Assistant license
Licensure and certification in good standing required
Ability to obtain licenses in requested states within 90 days of hire
Experience managing hypertension, hyperlipidemia, and primary and secondary prevention of atherosclerotic cardiovascular disease
Thorough understanding of cardiometabolic condition management, including disease processes, treatment modalities, medication management, and lifestyle modification
Comfortable using digital live portal chat and AI-powered clinical tools
Experience with STARS/HEDIS measures, coding clinical comorbidities, and identifying clinical care gaps
Virtual care experience preferred
Experience or strong interest in centralized results management, digital/chat-based patient engagement, and AI-enabled clinical workflows is a plus
Proficiency in English and Spanish preferred
Ability to work three 12-hour shifts per week, including Saturday and Sunday, from 8am–8:30pm EST
