About the role
Key responsibilities & impact- Conduct focused and thorough virtual assessments of patients with hypertension, hyperlipidemia, coronary artery disease, diabetes, and other chronic conditions
- Formulate diagnoses and individualized treatment plans, including medication management, lifestyle modifications, and monitoring recommendations
- Manage prescription refill inboxes and asynchronous medication refills according to protocols and guidelines
- Participate in centralized review, triage, and follow-up of laboratory and diagnostic results
- Use live portal chat and AI-assisted prescribing and inbox-triage tools
- Identify risk factors, comorbidities, and treatment contraindications
- Counsel patients on medication adherence and potential side effects
- Collaborate with PCPs, specialists, pharmacy, clinical nursing, social work, family members, and caregivers
- Use the electronic health information system for virtual visits and provide interface feedback
- Maintain accurate patient medical records in compliance with legal and ethical guidelines
- Participate in quality improvement and professional development
- Comply with applicable laws, regulations, patient privacy requirements, and telehealth standards
Requirements
What you’ll need- Advanced Practice Provider: Nurse Practitioner or Physician Assistant
- At least 3 years working in outpatient clinical practice
- At least 2 years concentrated in primary care or a relevant subspecialty with substantial hypertension and lipid-management focus
- Experience ideally including hypertension, hyperlipidemia, and primary and secondary prevention of atherosclerotic cardiovascular disease
- Bilingual in Spanish and English
- Nurse Practitioners: active, in-good-standing RN and APRN licenses
- Physician Assistants: active, in-good-standing Physician Assistant license
- Required role licensure and certification in good standing
- Ability to obtain licenses in requested states within 90 days of hire and obtain additional state licenses as needed
- Thorough understanding of cardiometabolic condition management, including disease processes, treatment modalities, medication management, and lifestyle modification
- Proficiency in English and Spanish preferred
- Familiarity with STARS/HEDIS measures, clinical comorbidity coding, and identifying care gaps
- Comfort using digital portal chat and emerging AI-powered clinical tools
- Virtual care experience preferred
- Experience or strong interest in centralized results management, digital/chat-based patient engagement, and AI-enabled clinical workflows is a plus
- Ability to work Monday–Friday 8-hour shifts, generally 8 a.m.–5 p.m. MST/PST preferred, with EST/CST hours considered
