About the role

Key responsibilities & impact

Conduct focused and thorough assessments of patients with hypertension, hyperlipidemia, coronary artery disease, diabetes, and other chronic health conditions
Perform virtual visits, expected to comprise 40–60% of clinical effort
Formulate diagnoses and individualized treatment plans, including medication management, lifestyle modifications, and monitoring recommendations
Manage a prescription refill inbox and queue according to established protocols and guidelines
Participate in centralized results management, including review, triage, and follow-up on lab and diagnostic results
Use digital live portal chat and AI-assisted prescribing and inbox-triage tools to improve medication renewal, results management, and patient communication
Identify risk factors, comorbidities, and treatment contraindications
Counsel patients on medication adherence and potential side effects
Collaborate with PCPs, specialists, pharmacy, clinical nursing, social work, family members, and caregivers
Use the company’s electronic health information system for visits and provide interface feedback
Maintain accurate, current patient medical records
Participate in quality improvement initiatives and ongoing professional development
Comply with applicable laws, regulations, patient privacy requirements, and telehealth standards

Requirements

What you’ll need

Nurse Practitioner or Physician Assistant advanced practice provider
Three or more years working in outpatient clinical practice
Minimum of two years concentrated in primary care or a related subspecialty with substantial hypertension and lipid-management focus
Experience ideally including hypertension, hyperlipidemia, and primary and secondary prevention of atherosclerotic cardiovascular disease
Nurse Practitioners must hold active, in-good-standing RN and APRN licenses
Physician Assistants must hold an active, in-good-standing Physician Assistant license
Role licensure and certification must be in good standing
Ability to obtain licensure in requested states within 90 days of hire and additional states as needed
Thorough understanding of cardiometabolic condition management, treatment modalities, medication management, and lifestyle modification
Comfortable using digital live portal chat and emerging AI-powered clinical tools
Experience or strong interest in centralized results management, digital patient engagement, and AI-enabled clinical workflows
Virtual care experience preferred
Proficiency in English and Spanish preferred
Ability and willingness to work Monday–Friday, 8-hour shifts, generally 8am–5pm MST/PST, with EST/CST hours considered
Knowledge of STARS/HEDIS measures, clinical comorbidity coding, and identifying clinical care gaps
