About the role

Key responsibilities & impact

Deliver empathetic clinical care through real-time and asynchronous chat and other responsive care channels
Manage concurrent patient conversations while maintaining clinical accuracy, timely documentation, and follow-through
Guide and oversee patients’ medical management using a holistic framework including the 5M approach
Collaborate with primary care providers, specialists, pharmacy, and specialty care teams
Coordinate care with patients’ family members and caregivers when appropriate
Assess symptoms, medication questions, lab results, and new health concerns; provide clinical guidance and follow-up
Escalate patients to telephone, video, in-person, urgent, or emergency care when appropriate
Maintain ownership of patient needs through resolution or documented handoff
Maintain accurate and timely EHR documentation in compliance with legal, ethical, and telehealth regulations
Improve patient experience and outcomes through digital workflows and proactive communication
Contribute to standardized responses, escalation pathways, digital workflows, and quality standards
Provide flexible clinical coverage across Responsive Care queues and on-call services
Occasionally support broader Devoted Medical service lines
Adhere to applicable laws, regulations, privacy requirements, and telehealth standards

Requirements

What you’ll need

Advanced Practice Provider (NP or PA) with three or more years of experience in primary care, house calls, internal medicine, family medicine, geriatrics, palliative care, and/or urgent care
Ability to review AI-generated responses for clinical accuracy and safety
Nurse Practitioners must hold active, in-good-standing RN and APRN licenses
Physician Assistants must hold an active, in-good-standing Physician Assistant license
Must be able to obtain licenses in requested states within 90 days of hire, beginning at offer-letter signing
Must obtain additional state licenses as needed
Ability and willingness to work 3x12s or 4x10s, with Saturday and Sunday coverage
Prior virtual care or telehealth experience preferred
Experience in real-time, queue-based, or high-volume telehealth clinical environments
Experience collaborating with interdisciplinary care teams and coordinating care remotely
Comfort managing complex or vulnerable patient populations in a digital-first setting
Flexibility and curiosity in a fast-paced, rapidly evolving environment
Must be located within one of the contiguous 50 states
