About the role

Key responsibilities & impact

Deliver empathetic clinical care through real-time and asynchronous chat and other responsive care channels
Manage concurrent patient conversations with accurate documentation and timely follow-through
Guide and oversee patients’ medical management using a holistic framework, including the 5M approach
Collaborate with primary care providers, specialists, pharmacy, and specialty care teams
Coordinate care with patients’ family members and caregivers when appropriate
Assess and provide clinical guidance and follow-up for symptoms, medication questions, lab results, and new health concerns
Escalate patients to telephone, video, in-person, urgent, or emergency care when chat is insufficient
Maintain ownership of patient needs through resolution or documented handoff
Maintain accurate and timely EHR documentation in compliance with legal, ethical, and telehealth regulations
Identify opportunities to improve patient experience and outcomes through digital workflows and proactive communication
Contribute to digital workflows, standardized responses, escalation pathways, and quality standards
Support broader Devoted Medical service lines as needed
Provide flexible clinical coverage across Responsive Care queues and on-call services
Adhere to applicable laws, regulations, patient privacy requirements, and telehealth standards

Requirements

What you’ll need

Advanced Practice Provider (NP or PA) with 3 or more years of experience in primary care, house calls, internal medicine, family medicine, geriatrics, palliative care, and/or urgent care
Ability to review AI-generated responses for clinical accuracy and safety
Nurse Practitioners: active and in-good-standing RN and APRN licenses
Physician Assistants: active and in-good-standing Physician Assistant license
Role licensure and certification in good standing required
Ability to obtain licenses in requested states within 90 days of hire, beginning at offer-letter signing
Willingness to work 4 ten-hour shifts per week
Availability for working hours of 8am-6pm MST/PST, with consideration for EST/CST hours
Prior virtual care or telehealth experience preferred
Experience in real-time, queue-based, or high-volume telehealth clinical environments
Experience collaborating with interdisciplinary care teams and coordinating care remotely
Comfort managing complex or vulnerable patient populations in a digital-first setting
Flexibility and curiosity in a fast-paced, rapidly evolving environment
Must be located within one of the contiguous 50 states
