About the role

Key responsibilities & impact

Deliver virtual telehealth acute care to members with emergent or critical illness
Evaluate and diagnose acute illnesses and complex medical conditions
Order and interpret diagnostic tests
Establish care plans, prescribe appropriate medications, and assess quality-of-care interventions
Assess social, home health, and durable medical equipment needs
Coordinate care with PCPs, specialists, pharmacy, clinical nursing, social work, family members, and caregivers
Use the electronic health information system for visits and provide feedback to improve the interface
Participate in weekend on-call coverage in certain geographies to support clinical nurses
Support new clinical or organizational programs and initiatives as the role evolves
Integrate innovative virtual care practices

Requirements

What you’ll need

At least three years of experience working as an Advanced Practice Provider
Experience preferably in acute settings, urgent care, emergency medicine, primary care, or internal medicine with heavy acute management
Willingness to work three 12-hour shifts per week, including Saturday and Sunday, from 8am–8:30pm EST
Nurse Practitioners must hold active, in-good-standing RN and APRN licenses
Physician Assistants must hold an active, in-good-standing Physician Assistant license
Required role licensure and certification must be in good standing
Ability to obtain licenses in requested states within 90 days of hire and obtain additional state licenses as needed
Experience in primary care, internal medicine, urgent care, emergency medicine, and/or geriatrics
Experience performing visits over telehealth video platforms
Experience managing acute or chronic disease exacerbations, including CHF, diabetic, COPD, and hypertensive emergencies
Bilingual Spanish/English is a plus
Strong desire to continue practicing acute care
Adaptability, eagerness to learn, flexibility in scope of practice, and willingness to embrace new challenges
