About the role

Key responsibilities & impact

Deliver virtual telehealth acute care to Devoted Health members with emergent or critical illness
Diagnose complex medical conditions
Order and interpret diagnostic tests
Establish care plans and prescribe appropriate medications
Assess members for quality-of-care interventions and social, home health, and DME needs
Coordinate care with PCPs, specialists, pharmacy, clinical nursing, social work, family members, and caregivers
Use the electronic health information system for visits and provide interface improvement feedback
Support weekend on-call coverage for clinical nurses in certain geographies
Support new clinical or organizational programs and initiatives as the role evolves
Integrate innovative virtual care practices

Requirements

What you’ll need

At least three years of experience working as an Advanced Practice Provider, preferably in acute settings
Experience in acute care, urgent care, emergency medicine, primary care, internal medicine, and/or geriatrics
For Nurse Practitioners: active and in-good-standing RN and APRN licenses
For Physician Assistants: active and in-good-standing Physician Assistant license
Required role licensure and certification in good standing
Ability to obtain licenses in requested states within 90 days of hire, beginning at offer-letter signing
Willingness to obtain additional state licenses as needed
Willingness to work four 10-hour shifts per week, including one weekend shift per month, from 11am–9pm EST
Experience performing visits over telehealth video platforms
Experience managing acute/chronic disease exacerbations, including CHF, diabetic, COPD, and hypertensive emergencies
Adaptability, eagerness to learn, and comfort working in a fast-paced, dynamic setting
Flexibility in scope of practice and willingness to embrace new challenges
