About the role
Key responsibilities & impact- Provide virtual primary care visits via telehealth, including comprehensive assessments, chronic disease management, preventive screenings, and acute care interventions.
- Diagnose and manage chronic conditions (e.g., hypertension, diabetes, COPD, heart disease) and develop individualized care plans to optimize patient health.
- Order and interpret lab tests, imaging, and other diagnostics, coordinating follow-up care as necessary.
- Manage medication prescriptions including controlled substances, titrations, and refills, ensuring patient adherence and safety.
- Provide health coaching, lifestyle modification guidance, and patient education to empower patients in managing their health.
- Identify patients needing referrals, or home-based services, and facilitate appropriate coordination with specialists or community resources.
- Utilize and provide feedback on our electronic health platform, contributing to its continuous improvement to enhance patient and provider experience.
- Participate in clinical case reviews, team huddles, and interdisciplinary collaborations to ensure high-quality, patient-centered care.
- Engage in continuous learning and improvement, staying up-to-date with best practices in virtual care and primary care management.
Requirements
What you’ll need- Active Advanced Practice Provider (APRN or PA) license in good standing in AL, AZ, AR, CO, FL, GA, HI, IL, IN, KY, MS, MO, NC, OH, PA, SC, TN, TX (must be willing to obtain additional licenses and a DEA certification within the first 90 days — support and reimbursement provided)
- Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
- Minimum 2 years of primary care, urgent care, or internal medicine experience, preferably with experience in telehealth or virtual care settings.
- Experience managing chronic conditions, preventive care, and acute illnesses via telehealth.
- Familiarity with value-based care models, population health management, and quality metrics (HEDIS, STAR ratings, etc.) is a plus.
- Comfortable with digital documentation and telemedicine platforms (experience with EHRs and virtual care workflows preferred).
- Active BLS certification at time of hire.
- Minimum of 6 months of experience working as a Nurse Practitioner in the D2Me program required.
- Shift times may vary in order to align with standard business hours (9:00 AM – 5:00 PM) in the patient’s local time zone. Flexibility is required.
