About the role

Key responsibilities & impact

Conduct patient assessments in Spanish
Provide virtual guidance, primarily through phone and video visits
Collaborate with a multidisciplinary team, including cardiology-trained nurse practitioners, pharmacists, social workers, and care coordinators
Completing remote (telephonic/video) visits to conduct nursing assessments
Provide comprehensive clinical education tailored to individual needs (disease process, medications, symptom monitoring, lifestyle counseling, contingency planning)
Developing care plans in partnership with members and their caregivers
Collaborate closely with our PCP partners and other clinical teams within Devoted Medical Group to coordinate care

Requirements

What you’ll need

An unrestricted, compact RN license
A minimum of 4 years of RN experience with at least 2 years in a non-ICU cardiac focused setting
A secure & private workspace with a strong & reliable internet connection.
Bilingual in English & Spanish.
Strong interpersonal and communication skills
Highly comfortable with technology (EMRs, messaging platforms, online portals, AI chat)
Ability to multi-task: listening, talking and typing all at the same time.
Team player mentality with a can-do attitude.
Comfortable with fast paced and evolving workflows
Willingness to work 8-5 in Mountain Time zone or later time zone (MT, PCT)
