About the role
Key responsibilities & impact- Complete and review required case record documentation
- Work with referral sources and market Agency Directed Services
- Provide support to families in the Agency Directed Services program
- Support nurses with case record documentation in identified office locations
- Create and maintain caregiver profiles and upload client packets in CareSmartz 360
- Prepare paperwork for intake and other visits
- Scan and upload approvals, denials, and appeal information into CareSmartz 360
- Work with finance to obtain appropriate service authorizations
- Speak with referral sources, potential referrals, and social services workers about services and Medicaid eligibility
- Participate in program growth, expansion, resource-identification, marketing, program, and team meetings
- Represent ADS at marketing events
- Identify and implement improvements in CareSmartz 360
- Assist personal care attendants with paperwork and visit scheduling
- Follow up with clients and families and document interactions in CareSmartz 360
- Participate in personal care attendant interviews, check references, prepare hiring documents, and conduct drug screens as needed
- Meet regularly with the program director regarding referrals, service provision, program needs, and service-delivery concerns
- Complete other duties within the scope of the program as assigned
Requirements
What you’ll need- Associate’s degree with experience working in human service programs or organizations serving individuals with disabilities or aging populations
- Comparable combination of professional and/or personal experience and education may be considered
- Knowledge of records administration techniques and procedures
- Knowledge of English language structure, spelling, composition, and grammar
- Proficiency with Microsoft Word, Excel, PowerPoint, Outlook, SharePoint, Zoom, virtual meeting applications, and program-specific software
- Knowledge of Medicaid waivers and agency directed services
- Knowledge of policies and guidelines from VDH, DMAS, DBHDS, HCBS, and DSS
- Ability to use standard office equipment and computer systems
- Ability to collect and analyze data
- Time management and ability to manage multiple priorities and complete accurate assignments on time
- Ability to prepare concise and accurate documents, reports, and correspondence
- Ability to communicate effectively orally and in writing
- Ability to follow verbal and written instructions and operating procedures
- Ability to develop objectives, evaluate effectiveness, and assess needs
- Critical-thinking and problem-solving ability in ambiguous or difficult situations
- Professionalism when handling sensitive and confidential information
- Valid driver’s license and at least five years of driving experience
- Driving record complying with insurer guidelines
- Satisfactory drug and alcohol screening and criminal background check after a conditional offer
- No barrier-crime convictions
- Legally authorized to work in the US
- Willing and able to commute to the Roanoke or Christiansburg offices
