About the role
Key responsibilities & impact- Conduct routine follow-up assessments in the homes of individuals receiving care.
- Serve as a resource to program staff pertaining to individuals’ medical needs.
- Conduct observational assessments during 90-day home visits.
- Complete required annual training(s) and participate in ongoing professional training and development.
- Report significant events concerning individuals to appropriate parties.
- Participate in staff and regular supervision meetings.
Requirements
What you’ll need- A Licensed Practical Nurse in good standing through the Virginia Board of Nursing is required.
- Previous experience doing home-based visits is preferred but not required.
