About the role
Key responsibilities & impact- Provide social work support to patients and their families at Sioux Falls and Moccasin Creek Dialysis Clinics
- Split coverage between the Sioux Falls and Moccasin Creek Dialysis Clinics
- Support and advocate for patients transitioning to life on dialysis
- Help patients understand their rights and responsibilities
- Guide patients in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease
- Build meaningful, long-term relationships with patients and their families in an outpatient setting
- Collaborate with and rely on the clinical team in a positive care environment
Requirements
What you’ll need- Licensed or registered in the state of practice as required by state regulations, such as LMSW or LCSW
- Meet all state-required regulations to practice in the dialysis setting
- Master's degree in Social Work (MSW) required with a specialization in clinical practice
- Demonstrated knowledge of government and private insurance programs
- Basic computer skills in Microsoft Word, Excel, PowerPoint, and Outlook
- Functional proficiency with DaVita-specific applications within 60 days
