About the role

Key responsibilities & impact
  • Coordinate and facilitate quality, cost-effective care for ESRD and CKD patients
  • Minimize fragmentation of the healthcare delivery system
  • Coordinate care between patients, families, physicians, providers, care teams, community resources, and the Regional Operations Manager
  • Integrate and manage care for late-stage CKD and ESRD patients
  • Assess patient needs and develop and manage individualized care plans
  • Coordinate services across interdisciplinary teams
  • Support patients through complex healthcare transitions
  • Manage clinical, financial, and patient satisfaction outcomes
  • Serve as a change agent and apply CQI techniques
  • Provide healthcare performance coaching
  • Adapt personal practice patterns to new/electronic processes and increased productivity expectations related to Capella implementation

Requirements

What you’ll need
  • ADN degree from an accredited school of nursing required; a three-year diploma from an accredited diploma program may be substituted
  • BSN preferred
  • Current Registered Nurse (RN) license in the state of practice required
  • Continuing education credits maintained as required by state of practice required
  • Minimum of five years’ experience in clinical nursing strongly preferred
  • Minimum of three years’ experience in renal nursing preferred
  • Case management experience required, or 3 or more years assessing patient needs, developing and managing individualized care plans, coordinating services across interdisciplinary teams, and supporting patients through complex healthcare transitions
  • Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferred
  • Knowledge and understanding of data and managing clinical, financial, and patient satisfaction outcomes
  • Experience and effectiveness in a change agent role
  • Knowledge and understanding of CQI techniques
  • Previous healthcare performance coaching experience required
  • Current CPR certification required
  • Ability to adapt to new/electronic processes and increased productivity expectations related to Capella implementation
  • Current driver’s license in the state employed, positive driving record, and ability to meet insurance coverage requirements
  • Basic computer skills and proficiency in MS Word and Outlook required
  • Functional proficiency with DaVita clinical software programs, including Capella, required within 90 days of employment
  • Home office with internet connectivity of at least 1MB upload and 1MB download speed required
