About the role

Key responsibilities & impact

Leading a team of IKC RNs who case manage dialysis and CKD patients
Supporting a team of non-clinical IKC care coordinators
Managing team performance, productivity, and creating a special place to come to work
Representing the IKC leadership team with other IKC leaders
Effectively managing Disease Management programs in designated market or geography
Reviewing and analyzing data to drive patient care outcomes in programs across the market
Building and managing effective relationships with internal and external clients
Providing clinical support to reporting Registered Nurse Case Managers
Managing all teammate employment/HR activity such as payroll, PTO, hiring, onboarding duties, and professional development
Ensuring compliance to all applicable regulations and DaVita policies
Monitoring risk management and quality assurance compliance
Achieving quality and metric goals in Integrated Kidney Care programs
Coordinating activities with DaVita IKC management team

Requirements

What you’ll need

Minimum of 3-5 years’ management experience leading and managing teams required
Demonstrates experience managing to data driven outcomes and process improvement required
Strong communication on medical management, operational, and business-related issues
Minimum of 1-2 years’ experience in clinical nursing leadership highly preferred
RN license preferred
Current CPR certification required
Intermediate computer skills and proficiency in MS Word, Excel, PowerPoint, and Outlook required
Home office with Internet connectivity at minimum of 1MB upload and 1MB download speed required
