Overall program and project management of CCP for All
Build the pipeline: Work with CS and Sales Ops to communicate the concept with clients, get them to sign up, and manage the waves of implementation (forecasting volume)
Lead new CCP for All Implementation projects including communicating with Client Success team and Client, building project plans, coordinating contracting and scripting approvals, gathering client-specific pathway information and design decisions
Work with Engineering and Salesforce teams to implement technical pieces and client-specific data
Work with reporting team to set up and deliver standard reporting and communicate updates to care coordination agents
Determine and communicate to managers when volume dictates additional agent resources
Identifies and resolves issues that would delay project timelines
Complete data audits on new clients to ensure Signify and Client reporting are accurate and update Care Coordination Reporting with weekly/monthly/quarterly metrics
Assist with other team projects with multi-client rollout plans (e.g. Pre-Visit Questionnaire, Referrals)
Requirements
2+ years of project management experience, preferably within a Healthcare Organization, Operations, or Technology function
Bachelor’s degree in Business, Supply Chain Management, Operations Management, Finance, or related field
Excellent problem solving abilities and a proactive approach to addressing operational challenges
Reporting and analytics experience with the ability to create detailed reports and dashboards
Knowledge of Salesforce is a plus
Experience with project management and workflow tools like SmartSheets, Jira, Asana, or Lucid Charts (preferred)
Project Management Professional (PMP) certification is a plus
Experience managing change and maintaining excellent relations with executives, customers, Operations and Product Management staff at all levels
