About the role
- Conduct telephonic outreach to members with diabetes to assess needs and close gaps in care
- Provide education and support to promote member engagement and self-management
- Identify and address social determinants of health impacting care
- Refer members to appropriate care management programs and community resources
- Collaborate with providers and interdisciplinary teams to coordinate care
- Apply nursing processes in compliance with company and regulatory standards
- Document interactions and care plans accurately in electronic medical records
- Consult with supervisors to resolve challenges and improve member outcomes
Requirements
- Minimum 3 years of clinical experience supporting individuals with chronic conditions
- Diabetes education experience is required
- Current, unrestricted Registered Nurse (RN) license; multi-state licensure preferred
- CDE or CDCES certification required
- Proficiency in Microsoft Office Suite (Outlook, Excel), EMR systems, and keyboarding
