About the role
- Collaborate with an interdisciplinary care management team to support members on their path to better health
- Conduct comprehensive medication reviews and telephonically outreach patients
- Identify and resolve medication related interventions
- Oversee pharmacy technicians for pharmacy benefit and precertification issues
- Participate in case conferences and plan sponsor requested formal clinical assessments
- Enhance clinical knowledge through rounds and collaboration with colleagues
Requirements
- Active pharmacist license in good standing in the state of residence
- 6+ years of demonstrated clinical pharmacy experience, including experience in a hospital or ambulatory care setting
- Current or past employment in a hospital and/or ambulatory care setting as part of a care management team
- Applies clinical knowledge to review medications and resolve different disease management scenarios
- Demonstrated ability to review and apply clinical literature/pharmacy resources
- Proficiency in utilizing and navigating multiple software programs concurrently
