About the role
- Field Case Managers are frontline advocates for members who cannot advocate for themselves.
Responsible for assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness.
Utilize best-in-class operating and clinical models serving dual eligible (Medicare & Medicaid) populations.
Collaborate with members, providers, and community organizations to address health care and social determinant needs.
Develop proactive plan of care, conduct evaluations using clinical tools/data, apply clinical judgment to reduce risk factors, conduct holistic assessments including claims and co-morbid conditions, refer to interdisciplinary team, present cases at conferences, comply with regulatory/company policies, use motivational interviewing to engage members.
Requirements
- 3+ years clinical practical experience as a Registered Nurse (RN)
Must have active and unrestricted Michigan RN licensure
Must be willing and able to travel up to 25%- 50% of the time in Wayne and Macomb Counties. Reliable transportation required. Mileage is reimbursed per our company expense reimbursement policy
Demonstrates proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint
Effective computer skills including navigating multiple systems and keyboarding
Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually
Excellent analytical and problem-solving skills
Effective communications, organizational, and interpersonal skills
Ability to work independently
Preferred: 2+ years of experience in case management, discharge planning and/or home health care coordination experience
Preferred: Behavioral health experience
Preferred: Bilingual in Spanish or Arabic
Preferred: Certified Case Manager
Additional national professional certification (CRC, CDMS, CRRN, COHN)
Education: Associate's Degree in Nursing (REQUIRED)
Bachelor's Degree in Nursing (PREFERRED)
