About the role
Key responsibilities & impact- Infuse patients in their homes and travel to and from patients’ homes
- Administer specialty medications via IV, subcutaneous injection, or other prescribed methods
- Monitor patients for adverse reactions or complications during and after infusions
- Assess medical history, allergies, medications, physical condition, and vital signs
- Identify and document infusion-therapy risks, side effects, and issues
- Educate patients and caregivers about medications, side effects, treatment expectations, and emergency management
- Schedule follow-up calls or visits and provide feedback for care-plan adjustments
- Maintain accurate infusion, patient-response, adverse-event, medication-administration, education, and follow-up records
- Collaborate with nurses, pharmacists, and pharmacy staff on patient care and treatment coordination
- Communicate treatment or patient concerns to the clinical nurse supervisor
- Prepare and handle medications safely and verify medication and dosage
- Follow sterile techniques, infection-control guidelines, and safety protocols
- Monitor specialty-medication and infusion-supply inventory
- Support insurance verification or authorization processes
- If IgCN-certified and assigned, mentor nurses, support training, observe clinical practice, assess competency, and document mentorship activities
Requirements
What you’ll need- Licensed Registered Nurse (RN)
- College degree/certification required or equivalent experience
- 2 years of RN experience required
- 1 year of experience accessing PIV lines, ports, and maintaining PICC lines required
- 1 year of ER/ICU/infusion experience preferred
- 1 year of infusion experience in a hospital or Home Health setting preferred
- Must be willing to travel up to 2 hours out from Scranton, PA to see patients in their homes
- Successful completion of a drug screen prior to employment required
- Ability to perform designated technical, clerical, and clinical home-care tasks
- IgCN certification applies to additional mentorship responsibilities when active
