About the role

Key responsibilities & impact
  • Provide remote operational support to Care Managers across centers by backfilling vacancies as needed.
  • Conduct proactive outreach to patients to complete missed care plans.
  • Identify patient care requirements by establishing personal rapport with patients.
  • Leverage patient data from dashboards and reports to identify trends and improve patient engagement.
  • Accurately document patient care services.
  • Address patient needs related to opioid use disorder.
  • Establish individualized patient care goals in collaboration with patients.
  • Schedule and reschedule patient appointments.
  • Monitor patients’ clinical status.
  • Assist physicians and clinicians with patient onboarding and orientation.
  • Maintain patient information confidentiality and privacy.

Requirements

What you’ll need
  • At minimum, High School Diploma or GED required.
  • Experience in a fast-paced environment, servicing many clients, is preferred.
  • Experience in healthcare, addiction medicine, community health, care coordination preferred.
  • Experience with building individualized care plans is a plus.
  • Knowledge of compliant documentation practices.
  • Lived Addiction experience is a plus.
  • Can-do attitude and willingness to think beyond the status quo.
  • Mission-driven with a passion for helping patients live better, stronger lives.
  • Strong organizational and time management skills with the ability to prioritize multiple tasks in a fast-paced environment.
  • Strong critical thinking and problem-solving skills with the ability to identify barriers to care and implement appropriate interventions.
