About the role
Key responsibilities & impact- Provide remote operational support to Care Managers across centers by backfilling vacancies as needed.
- Conduct proactive outreach to patients to complete missed care plans.
- Identify patient care requirements by establishing personal rapport with patients.
- Leverage patient data from dashboards and reports to identify trends and improve patient engagement.
- Accurately document patient care services.
- Address patient needs related to opioid use disorder.
- Establish individualized patient care goals in collaboration with patients.
- Schedule and reschedule patient appointments.
- Monitor patients’ clinical status.
- Assist physicians and clinicians with patient onboarding and orientation.
- Maintain patient information confidentiality and privacy.
Requirements
What you’ll need- At minimum, High School Diploma or GED required.
- Experience in a fast-paced environment, servicing many clients, is preferred.
- Experience in healthcare, addiction medicine, community health, care coordination preferred.
- Experience with building individualized care plans is a plus.
- Knowledge of compliant documentation practices.
- Lived Addiction experience is a plus.
- Can-do attitude and willingness to think beyond the status quo.
- Mission-driven with a passion for helping patients live better, stronger lives.
- Strong organizational and time management skills with the ability to prioritize multiple tasks in a fast-paced environment.
- Strong critical thinking and problem-solving skills with the ability to identify barriers to care and implement appropriate interventions.
