About the role
Key responsibilities & impact- Provides individual counseling to highly complex patients
- Conducts clinical assessments to appropriately assess patient’s treatment needs
- Designs and updates the patient’s treatment plan, including care coordination, referrals, and discharge planning
- Manages assigned caseload and compliance with clinical treatment requirements
- Collaborates with nurses and providers to provide holistic treatment services
- Facilitates urine drug screening for patients
Requirements
What you’ll need- General experience working with underserved populations in a clinical setting.
- Lived addiction experience is a plus.
- Qualified candidates will hold one or more of the following certifications/licenses: Bachelor's degree in human services or a related field.
- Certified/licensed in addiction counseling in the state of SC is preferred.
- Must be willing/able to get certified in SC.
