About the role

  • Connect with patients monthly to address any needs and provide the resources to fulfill those needs.
  • Connect and maintain relationships with community partners for patient resources.
  • Work closely with a dedicated team to provide exceptional patient care.
  • Participate in community awareness/engagement events to promote Crossroads, mission, principles, and approach to addiction treatment.
  • Build and support individualized care plans and coordinate resources like housing, food, and healthcare.
  • Document interactions and care plans in compliance with documentation practices.

Requirements

  • At minimum, High School Diploma or GED required.
  • Experience in a fast-paced environment, servicing many clients, is preferred.
  • Experience in healthcare, addiction medicine, community health, care coordination preferred.
  • Experience with building individualized care plans is a plus.
  • Knowledge of compliant documentation practices.
  • Lived Addiction experience is a plus.
  • Ability to work Monday–Friday (Uniontown) or rotating weekends (Connellsville) as scheduled.
