About the role
- Connect with patients monthly to address needs and provide resources to fulfill those needs
- Connect and maintain relationships with community partners for patient resources
- Work closely with a dedicated team to provide exceptional patient care
- Participate in community awareness/engagement events to promote Crossroads, mission, principles, and addiction treatment approach
- Build and support individualized care plans and maintain compliant documentation practices
- Travel to primary and secondary clinic locations as required
- Provide front-line assistance to help patients find housing, food, healthcare, and other community resources
- Attend community events and outreach (cook outs, fairs, parades, school awareness events)
Requirements
- At minimum, High School Diploma or GED required
- Experience in a fast-paced environment, servicing many clients, is preferred
- Experience in healthcare, addiction medicine, community health, care coordination preferred
- Experience with building individualized care plans is a plus
- Knowledge of compliant documentation practices
- Lived Addiction experience is a plus
- Ability to work expected hours (~37-40 hours per week) and varied clinic hours (6:30 AM – 9:00 PM)
- Ability and willingness to travel within a localized geographical area to primary and secondary work locations (Greensburg, Monroeville, Latrobe, Wilmerding, PA)
