About the role
- Day in the Life: Provides individual counseling to highly complex patients
- Conducts clinical assessments to appropriately assess patient’s treatment needs
- Designs and updates the patient’s treatment plan, including care coordination, referrals, and discharge planning
- Manages assigned caseload and compliance with clinical treatment requirements
- Collaborates with nurses and providers to provide holistic treatment services
- Facilitates urine drug screening for patients
- Outpatient, office-based clinics across multiple states (GA, KY, NJ, NC, PA, TN, TX, VA) and related care delivery
- Schedule and Hours: early morning schedule with early afternoon out times; Saturday rotation; 35 hours/week; 4:45 AM – 1:00 PM, Mon-Fri
Requirements
- General experience working with underserved populations in a clinical setting.
- Lived addiction experience is a plus.
- Must have an LCAS (with a CS preferred)
