About the role
Key responsibilities & impact- Patient Outreach & Nutritional Counseling: Manage an assigned panel of 300-350 patients and conduct required monthly nutritional outreach and support. Conduct 50-60+ daily phone calls with patients, providing nutritional care plan support and health coaching tailored to their specific conditions. Maintain call quality standards and professional communication in every patient interaction; pass regular QA call reviews.
- Nutrition Planning & Patient Education: Educate patients on nutrition concepts, dietary modifications, and weight management strategies. Simplify complex nutritional information for diverse patient populations. Reinforce provider-directed nutritional guidance and support patient adherence to established care plans.
- Care Coordination & Collaboration: Collaborate with clinical teams, providers, and caregivers to identify and address nutritional and clinical needs. Support care coordination goals by staying on schedule with monthly outreach targets. Escalate patient concerns or changes in condition to appropriate clinical staff per established protocols.
- Documentation & Compliance: Document all patient interactions in real time using the care coordination platform. Maintain HIPAA compliance in all patient communications and documentation. Ensure accurate documentation to support care coordination goals.
Requirements
What you’ll need- Minimum 2+ years of experience in a healthcare role such as care coordination, health coaching, Medical Assistant
- Must hold a nationally recognized medical assistant certification (CMA, RMA, CCMA, NCMA, CMAC, or equivalent state-issued MA-C)
- OR Active Licensed Practical Nurse or Licensed Vocational Nurse license: Must have a multi-state compact license.
- Minimum 2+ years of clinical nutrition counseling or dietary care coordination experience.
- 2+ years of Electronic Medical Records (EMR) and computer navigation experience.
- Demonstrated ability to educate patients and simplify complex nutritional concepts.
- HIPAA-compliant home office setup with minimum 50 Mbps download / 5 Mbps upload internet capability.
- Must be legally authorized to work in the United States.
