Key responsibilities & impact

Provide in-person and telephonic Medical Case Management
Utilize medical and nursing knowledge to discuss treatment plans
Conduct assessments, planning, implementation, and evaluation of patient’s progress
Evaluate treatment plans for appropriateness, medical necessity, and cost effectiveness
Attend doctors, home visits, and discharge planning conferences
Implement care such as delivering durable medical equipment and nursing services

Requirements

What you’ll need

Experience as an RN Medical Case Manager is ideal
Clinical background in orthopedics, neurology, or rehabilitation preferred
Graduate of accredited school of nursing
Current RN Licensure in state of operation
Certification as a CCM, CIRS, or other Case Management certifications preferred
Valid driver's license and reliable transportation required for local travel
