About the role

Key responsibilities & impact
  • Lead, supervise, and support provider teams, ensuring clinical quality and regulatory compliance.
  • Manage hiring, training, coaching, and performance evaluation for direct reports.
  • Strategically deploy clinical resources using data-driven KPIs to achieve market and organizational goals.
  • Develop and nurture partnerships with local clinical communities, health plans, and other key stakeholders.
  • Champion our value-based, patient-centered care model for the frailest and most vulnerable populations.
  • Deliver clinical oversight for patient care, including annual wellness visits, urgent care, care transitions, and continuity care.
  • Participate in care team huddles, providing expert clinical guidance to drive better outcomes and overall performance improvements.
  • Ensure compliance with all medical and operational policies, protocols, and guidelines.
  • Support direct patient care and on-call activities as needed.

Requirements

What you’ll need
  • Graduate of an accredited medical school; completion of Internal Medicine or Family Medicine residency.
  • Active medical licensure in Florida and eligibility for Kentucky licensure.
  • Board certification in Internal Medicine or Family Medicine required; additional certification in Geriatric Medicine or Palliative Care is desirable.
  • Active DEA and state-specific controlled substance registration.
  • Minimum 4 years post-residency clinical experience, with leadership and team management expertise.
  • Proven ability to build and inspire high-performing teams.
  • Experience working with health plans and health systems; managed care/value-based care experience preferred.
  • Expertise in caring for aging adults and patients with complex needs, especially in home-based setting.
  • Strong clinical analytical skills; adept with data analytics and healthcare software.
  • Self-starter with proactive, accurate, and efficient work style.
  • Licensed for a minimum of 5 years.
  • No vehicle-related suspensions, DUI, reckless driving, leaving the scene of an accident, vehicular manslaughter, felony convictions, or 3 or more moving violations within the last 5 years.
  • No more than 2 moving violations or 1 at-fault accident in the last 3 years.
