About the role

Key responsibilities & impact

Responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation
Establishes patient chart upon initial referral and maintains documentation until discharge
Facilitate flow of patient care from initial consultation to discharge
Submit and track patient billing and maintain patient metrics
Communicate effectively with Palliative Care service members, patients, families, and the community

Requirements

What you’ll need

High school diploma or GED required
Two (2) to three (3) years of related experience highly preferred
Experience as a Licensed Practical/Vocational Nurse preferred
Strong written and verbal communications
