  • ensuring effective utilization of treatment resources to improve patient outcomes
  • caring for a group of complex patients requiring an initial assessment and ongoing monitoring of their condition
  • developing and monitoring the patient's clinical pathway
  • collaborating with healthcare team members
  • conducting initial assessments and coordinating plans of care
  • tracking patient progress, articulating the role of Case Manager
  • reviewing aggregate variance and determining actions
  • precepting new employees as assigned
  • exploring strategies to reduce lengths of stays and resource consumption

Requirements

What you’ll need
  • Minimum Associate level nursing degree in an accredited program
  • prefer Bachelor of Science in Nursing
  • Minimum one (1) to two (2) years of nursing practice in medical/surgical, acute, critical care, or hospital care required
  • Experience in a home health or hospice setting preferred
  • Ability to compute rate, ratio, and percentage
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
  • Ability to write reports, business correspondence, and procedure manuals
  • Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties
  • Ability to understand, read, write, and speak English
  • Demonstrates clinical competency, evidence of nursing leadership, and coordinates the nursing care of a specific group of patients.
  • Knowledge of community resources and third-party payors
  • Knowledge of state and federal rules and regulations for Medicare and Medicaid and other regulatory agencies.
  • Current RN license in the state of Ohio
  • CPR certification
