About the role

Key responsibilities & impact

Assess patient and family needs
Provide professional nursing care grounded in the full nursing process
Work closely with Interdisciplinary Group (IDG) to develop individualized plans of care
Regularly reevaluate patient/family/caregiver needs and revise the Plan of Care as necessary
Initiate the Plan of Care and make necessary revisions based on patient status and needs
Develop care plans based on nursing diagnosis and incorporate palliative nursing actions
Participate in on-call duties as defined by the on-call procedures
Complete, maintain, and submit accurate and relevant clinical notes regarding patient’s condition and care given

Requirements

What you’ll need

Associate or Bachelor's degree from an accredited School of Nursing
1 year experience practicing as a registered nurse preferred
Experience as a Hospice CNA or Hospice LPN will be considered
Current unencumbered (State of Employment) License as a Registered Nurse
Within 30 days of hire - National Provider BLS - American Heart Association (Vendor Managed)
