About the role

  • Supervises on-call nurses during weekend hours: planning, assigning, and directing work as outlined by program Hospice Director of Clinical Services (DCS) or Hospice Area Executive of Clinical Operations (ACO).
  • Reads and incorporates into practice the requirements of the Hospice Conditions of Participation for Medicare, applicable accreditation standards, and state or other rules and regulations as applicable to the program.
  • Oversees the consultative process between the on-call Medical Directors assisting in requesting appropriate care orders and attendance to identify patient needs if the patient’s attending physician is not available.
  • Oversees Cares Program, that all patients admitted Acuity 1, assign visits as needed, follow up visits completed.
  • Acts as Region Administrator on weekends - addresses problems and ensures timely response for all programs.
  • Answers administrative questions and contacts the Hospice Area Executive of Clinical Outcomes directly (not DCS) for any serious issues that arise.
  • Models desired behaviors including timeliness, punctuality, attendance, collaboration, open communication, and equitable treatment.
  • Documents in the patient’s clinical record when required; demonstrates proficiency with documentation requirements.
  • Actively collaborates regarding QAPI activities and provides feedback and input into Performance Improvement.
  • Participates in performance improvement programs.
  • Communicates proactively when schedule changes are needed.
  • Analyzes and reports on quality metrics including Hospice Item Set, CAHPS, and symptom management outcomes.
  • Complete M and M’s for HIS, HARs, and other items as directed by the Hospice Area Executive of Clinical Operations, Hospice Director of Quality Outcomes, or Hospice Regional Executive of Clinical Operations.
  • Supervises patient and family care as specified by the plan of care, assessing appropriateness, continuity, service, and quality of care.
  • Models and facilitates communication between team members including medical records, business office, intake, clinical team, and leadership.
  • Completes and/or delegates post-admissions calls.
  • Approves any Out of Formulary items during weekend hours for the management of pharmacy, medical supplies, durable medical equipment, mileage, and labor for program to meet budget expectations.
  • Functions as the alternate administrator for the licensure of the program based on state requirements.
  • Ensures maximum utilization of resources.
  • Acts as back-up for Intake-HCHB entry, and insurance verification; in addition to, order entry for emergent PRN visits as needed.
  • Oversees all new referrals are admitted same/next day when possible.
  • Collaborates with the on-call team to overcome obstacles for admission.
  • Validates 1st and 2nd step ladders for new admits to schedule 24-hour post-admission visits are completed by the program on-call team or completes ladders as needed.
  • Monitors electronic referral systems for incoming referrals.
  • Conducts monthly on-call meetings in collaboration with program and Area leadership.
  • Performs other duties as assigned.

Requirements

  • Associate or Bachelor's degree in Nursing required.
  • Minimum of three (3) years in healthcare required, preferably in a nursing facility, home health or hospice setting.
  • Experience in leadership or management strongly preferred.
  • Experience with Electronic Medical Record systems a strong plus.
  • Understanding and acceptance of hospice philosophy.
  • Must be of high integrity including maintenance of confidential information including all personnel and patient records.
  • Ability to work a flexible schedule, to include some evenings and weekends.
  • Must be able to travel.
  • Strong leadership, organizational and interpersonal skills.
