About the role
- Maintain and enhance Healthcare Home tracking systems to support care pathways for chronic conditions (e.g., Tobacco Cessation, BMI management)
- Cross-check data across multiple systems to ensure accuracy and proper implementation
- Provide essential reporting for Healthcare Home leadership and quality initiatives
- Analyze program measures and create charts, summaries, and presentations for senior leadership and HCH Directors
- Prepare and distribute data for special projects such as Antipsychotic Medication Adherence and primary care collaboration initiatives
- Generate audit-ready reports and assist in compliance monitoring
- Evaluate organization-wide and region-specific outcomes to identify trends or concerns
- Support ongoing extraction, interpretation, verification, and quality checking of data
- Contribute to monthly reporting efforts by gathering and validating key outcome information
Requirements
- High School/GED required
- Enjoy working with complex data and turning it into clear, actionable insights
