Support and coordinate the mental and physical health care of patients on an assigned caseload with the patient's medical provider and other mental health providers when appropriate.
Provide direct patient care on a short-term basis.
Support psychotropic medication management.
Serve as a core member of the collaborative care team including the patient's medical provider and psychiatric consultant, and the larger primary care or medical team.
Requirements
Master’s Degree, Ph.D., or PsyD in a mental health, social work, counseling, or related behavioral health field is necessary.
Must hold an active and in good standing license such as Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), or Licensed Clinical Psychologist (LCP).
California Children Services (CCS) paneling is required within six months of hire.
Minimum: 1 year licensed experience in Behavioral Health.
Preferred: 3-4 years of related experience in a healthcare setting.
