About the role

Key responsibilities & impact

Conduct comprehensive assessments to identify clients' strengths and needs
Develop individualized care plans that address specific needs, goals, and preferences
Conduct outreach and intakes in encampment settings
Connect clients with appropriate community resources and coordinate services
Advocate for clients to ensure their rights and needs are respected
Offer ongoing emotional support and guidance
Monitor progress and outcomes, adjusting care plans as needed
Foster community integration through access to resources and peer support networks
Complete consistent and daily documentation

Requirements

What you’ll need

1-3 years of experience working with individuals experiencing homelessness, substance misuse, or mental health challenges
Knowledge of trauma-informed care, harm reduction principles, and evidence-based practices in substance abuse treatment and mental health
Familiarity with community resources, housing programs, and support services for homeless individuals
Valid driver's license and reliable transportation for home visits and client transportation
Excellent communication and interpersonal skills, with the ability to establish rapport and trust with clients from diverse backgrounds
Strong organizational and time management abilities to effectively handle a caseload and meet deadlines
Ability to work under pressure and respond to deadlines without sacrificing quality
Empathy, patience, and the ability to remain calm and supportive in challenging situations
Proficiency in documentation and record-keeping
