About the role
- Conduct comprehensive assessments to identify clients' needs, including substance misuse issues, mental health concerns, housing barriers, and other challenges affecting their well-being
- Develop individualized care plans in collaboration with clients that address their specific needs, goals, and preferences
- Ensure plans are realistic, achievable, and aligned with available resources
- Connect clients with appropriate community resources, such as substance abuse treatment programs, mental health services, medical care, and vocational training
- Serve as a strong advocate for clients, ensuring their rights and needs are respected and met
- Offer ongoing emotional support and guidance to clients, helping them navigate challenges, build resilience, and develop healthy coping mechanisms
- Regularly assess clients' progress towards their goals, adjusting care plans as needed
- Foster community integration by facilitating access to community resources, social activities, and peer support networks
Requirements
- Combination of education and 2 years of experience working with homeless populations to supplement our preference of a bachelor's degree in social work, psychology, counseling, or a related field
- Valid driver's license and reliable transportation for home visits and client transportation
- Previous experience working with individuals experiencing homelessness, substance misuse, or mental health challenges
- Knowledge of trauma-informed care, harm reduction principles, and evidence-based practices in substance abuse treatment and mental health
- Familiarity with community resources, housing programs, and support services for homeless individuals
- Excellent communication and interpersonal skills, with the ability to establish rapport and trust with clients from diverse backgrounds
- Strong organizational and time management abilities to effectively handle a caseload and meet deadlines
- Empathy, patience, and the ability to remain calm and supportive in challenging situations
- Proficiency in documentation and record-keeping.
