Temporary Case Manager Team Lead
Community Economic Defense ProjectTemporary Case Manager Team Lead providing essential support to individuals experiencing homelessness and housing instability. Responsibilities include developing care plans and connecting clients to community resources.
About the role
Key responsibilities & impact- Conduct comprehensive assessments to identify clients' needs, including substance misuse issues, mental health concerns, housing barriers, and other challenges affecting their well-being
- Develop individualized care plans in collaboration with clients that address their specific needs, goals, and preferences
- Ensure plans are realistic, achievable, and aligned with available resources
- Connect clients with appropriate community resources, such as substance abuse treatment programs, mental health services, medical care, and vocational training
- Serve as a strong advocate for clients, ensuring their rights and needs are respected and met
- Offer ongoing emotional support and guidance to clients, helping them navigate challenges, build resilience, and develop healthy coping mechanisms
- Regularly assess clients' progress towards their goals, adjusting care plans as needed
- Foster community integration by facilitating access to community resources, social activities, and peer support networks
Requirements
What you’ll need- Combination of education and 2 years of experience working with homeless populations to supplement our preference of a bachelor's degree in social work, psychology, counseling, or a related field
- Valid driver's license and reliable transportation for home visits and client transportation
- Previous experience working with individuals experiencing homelessness, substance misuse, or mental health challenges
- Knowledge of trauma-informed care, harm reduction principles, and evidence-based practices in substance abuse treatment and mental health
- Familiarity with community resources, housing programs, and support services for homeless individuals
- Excellent communication and interpersonal skills, with the ability to establish rapport and trust with clients from diverse backgrounds
- Strong organizational and time management abilities to effectively handle a caseload and meet deadlines
- Empathy, patience, and the ability to remain calm and supportive in challenging situations
- Proficiency in documentation and record-keeping.
