About the role
Key responsibilities & impact- Provide high-quality primary care in a high-touch, person-centered care model
- Diagnose, treat, and manage acute episodic and chronic illness
- Conduct wellness visits and detect changes early
- Provide timely treatment and health education
- Develop and communicate treatment plans to residents, providers, and caregivers
- Order and interpret laboratory and diagnostic testing and adjust treatment plans
- Maintain compliant documentation in the electronic medical record
- Collaborate with primary care providers, interdisciplinary teams, and facility leadership
- Support value-based care strategic initiatives
- Provide an excellent customer experience
- Follow professional scope and standards of practice, state and federal regulations, guidelines, and laws
- Coordinate care with providers, specialists, and telemedicine providers
- Complete required monthly, quarterly, annual, and acute visits
- Participate in family conferences and provide advanced care planning and education
- Address pharmacy recommendations and pharmacist communications
- Provide face-to-face coverage in assigned facilities during work hours
Requirements
What you’ll need- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
- National certification from AANP or ANCC
- Active, unrestricted state license
- Active DEA license
- US work authorization
- 2+ years of clinical experience as a Nurse or Nurse Practitioner in a geriatric, adult, or family practice setting
- Ability to work in a flexible environment and schedule
- Passion for providing healthcare to senior and low-income populations
- Strong oral and written communication skills
- Detail-oriented, with ability to work independently and in a team setting
- Customer-service approach with teams and residents
- Strong time-management skills
- Ability to prioritize and adhere to competing deadlines while achieving goals
- Providers must be present in assigned facilities during work hours
- Preferred: training or clinical experience in geriatrics
- Preferred: 2+ years of clinical experience as a Nurse Practitioner
- Preferred: experience with coding, ICD-10, and HCC
- Preferred: experience or training in metrics- and results-driven healthcare models
- Preferred: experience working in value-based care models
- Preferred: training or experience in geriatric, assisted living, and/or skilled nursing settings
- Preferred: experience in collaborative healthcare settings
- Preferred: training or experience in outpatient primary care settings
