About the role

Key responsibilities & impact
  • Provide high-quality primary care in a high-touch, person-centered care model
  • Diagnose, treat, and manage acute episodic and chronic illness
  • Conduct wellness visits and detect changes early
  • Provide timely treatment and health education
  • Develop and communicate treatment plans to residents, providers, caregivers, and others
  • Order and interpret laboratory and diagnostic testing and adjust treatment plans
  • Maintain compliant documentation in the EMR software
  • Collaborate with operations, clinical leadership, PCPs, interdisciplinary teams, and specialists
  • Support value-based care initiatives
  • Provide an excellent customer experience
  • Complete required monthly, quarterly, annual, and acute visits
  • Participate in family conferences and provide advanced care planning and education
  • Address pharmacy recommendations and pharmacist communications
  • Coordinate care with telemedicine providers
  • Provide face-to-face coverage at assigned facilities during work hours
  • Follow professional standards, state and federal regulations, guidelines, and laws

Requirements

What you’ll need
  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
  • National certification from AANP or ANCC
  • Active, unrestricted state license
  • Active DEA license
  • US work authorization
  • 2+ years of clinical experience as a Nurse or Nurse Practitioner in a geriatric, adult, or family practice setting
  • Ability to work in a flexible environment and schedule
  • Passion for providing healthcare to senior and low-income populations
  • Strong oral and written communication skills
  • Detail-oriented and able to work independently and in a team setting
  • Customer service approach with teams and residents
  • Strong time management skills
  • Ability to prioritize and meet competing deadlines while achieving goals
