About the role
Key responsibilities & impact- Provide remote nursing support to clinic teams by focusing on RN tasking, lab reports, patient communication, and care coordination.
- Complete nurse lab reports in the morning and ensure timely review, follow-up, and documentation.
- Manage the clinic task box and patient portal in a timely and accurate manner, including patient outreach, education, and coordination with providers and care teams.
- Support abnormal lab result workflows and treat per protocol when appropriate.
- Help with outreaching any walk-in patients requiring triage when asked.
- Collaborate with providers, pod teams, and remote nursing team to support and help facilitate patient care.
- Occasionally assist remotely with other sites with high task volumes.
- Attend to the needs of the team including injections, medication administration and patient education.
- Conduct independent Nurse Visits such as wound care, managing INRs, Hypertension visits, UTIs, etc.
- Evaluate patients in collaboration with the provider and treat patients with acute needs to help create access for patients.
- Assess walk-in Triage patients.
- Build teamwork by attending huddles before the start of shifts to review any anticipated nurse needs during that shift.
Requirements
What you’ll need- Bilingual in English and Spanish
- Active Colorado RN license
- Live within commuting distance of 1701 W. 72nd Ave, Denver, CO
