Pharmacy Clinical Coordinator – Critical Care

- Support the inpatient pharmacy team at Lutheran Hospital.
- Participate in daily ICU rounds as the clinical pharmacy expert.
- Provide comprehensive medication management for critically ill patients.
- Guide complex pharmacotherapy decisions and collaborate closely with physicians, nursing, and interdisciplinary teams.
- Oversee and coordinate clinical pharmacy services, partner on strategic initiatives, and implement service improvements.
- Ensure compliance with practice agreements and protocols.
- Contribute to committees and governance efforts.
Requirements
What you’ll need- Doctor of Pharmacy degree from an American Association of Colleges of Pharmacy (AACP) accredited university
- Current state licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire.
- For individuals required to administer immunizations, current Basic Life Support (BLS) through American Heart Association (AHA) or American Red Cross and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire are required.
- For individuals required to respond to adult or pediatric cardiopulmonary arrest events, current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) Certification through American Heart Association (AHA), or certification obtained within one (1) year of hire, is required.
- Completion of a post-graduate year 2 residency (PGY2) plus a minimum of 1 year of experience providing pharmaceutical care in the relevant specialty area OR Completion of a post-graduate year 1 (PGY-1) residency plus a minimum of 3 years of experience providing pharmaceutical care in the relevant specialty area OR minimum of 5 years of experience providing pharmaceutical care in the relevant specialty area
- Previous leadership experience
- Board certification through the Board of Pharmacy Specialties that aligns to practice area strongly preferred at time of hire and required within one year
- Clinical expertise in acute/critical care pharmacotherapy.
