About the role

Key responsibilities & impact

Contact patients to introduce CCM and RPM services and explain program benefits.
Guide patients through the enrollment and consent process.
Document all patient interactions and enrollment activities accurately within EMR systems.
Ensure compliance with CMS, HIPAA, and internal documentation standards.
Answer patient questions regarding care coordination services and program expectations.
Collaborate with healthcare providers and internal teams to maintain smooth communication and patient transitions.
Maintain organized workflows while managing daily outreach goals and follow-up tasks.
Support positive patient experiences through empathy, professionalism, and clear communication.

Requirements

What you’ll need

Active, nationally recognized CMA or RMA certification.
Current Basic Life Support (BLS) certification.
Proven experience working directly with CCM and/or RPM programs.
Strong proficiency with EMR systems and Google Suite.
Dedicated, HIPAA-compliant home office environment that is private and distraction-free.
Strong time management, organizational, and independent work skills.
Excellent verbal and written communication abilities with strong patient empathy.
Previous experience in care coordination or transitional care management (Nice to Have).
Strong understanding of CMS regulations, healthcare documentation, and quality reporting standards (Nice to Have).
Bilingual fluency in English and Spanish (Nice to Have).
