About the role
- Support patients throughout Middlesex and Essex Counties, MA
- Manage a panel of rising and high intensity members for chronic disease and behavioral health care
- Collaborate with members to create a care plan and oversee progress
- Conduct self-efficacy and condition-specific screeners during assessment
- Perform clinical examinations and participate in case conferences
- Develop and monitor care plans in collaboration with members
- Conduct regular clinical visits and follow-ups per program guidelines
- Collaborate with care team for health maintenance and chronic disease management
- Perform medication reconciliation and documentation
Requirements
- 3+ Years of experience
- Graduate of an accredited school of nursing (R.N.)
