About the role

Key responsibilities & impact
  • Supervise the operations of the division's Supportive Housing (SH) and Homemakers and Nurses Services (HMNS) program
  • Implements detailed plans and recommends policies/procedures regarding program specific requirements
  • Supervises, motivates and trains assigned staff, ensuring effective teamwork and high standards of work quality
  • Monitors and evaluates staff performance and approves salary increments
  • Investigates concerns and complaints brought forward by clients and families
  • Engages divisional resources; consults with Legal, Ombudsman, Human Rights offices
  • Plans, develops and directs the effective delivery of services to clients
  • Manages and maintains collaborative partnerships with contracted agencies and community organizations
  • Ensures accurate maintenance of clients' healthcare records

Requirements

What you’ll need
  • Post-secondary education with a degree in Nursing and/or another professional discipline pertinent to the job function
  • Considerable experience planning, directing and co-ordinating community-based programs
  • Considerable experience motivating, leading and supervising regulated and unregulated staff in a unionized community setting
  • Experience managing complex client situations involving high risk seniors and/or vulnerable populations in a community setting
  • Experience working collaboratively with external partners, such as community groups, community service agencies, hospitals, staff, clients and families
  • Highly developed interpersonal, communication (written and verbal), negotiation, conflict resolution skills
  • Ability to develop, deliver and evaluate programs with a quality improvement focus
  • Ability to navigate computer systems, software and applications such as Microsoft Word and Excel
  • Sensitivity and understanding of the diversity of knowledge, languages, literacy levels, culture and ethno-racial background in communities throughout Toronto
