About the role

Key responsibilities & impact

Help build the operational backbone of Circadia's growing Home Health program
Coordinate daily schedules for field-based nurse practitioners
Build and optimize daily schedules for nurses conducting in-home or virtual visits
Coordinate geographically efficient routes to minimize travel time and maximize patient visits
Schedule new patient visits and follow-up appointments
Monitor provider schedules throughout the day and quickly adjust for changes
Collaborate with operations, nurse practitioners, and care teams to support seamless daily operations
Support onboarding of new providers by integrating them into efficient scheduling workflows

Requirements

What you’ll need

2+ years of scheduling experience in Home Health, Home Care, Hospice, or another mobile healthcare setting
Experience scheduling field clinicians traveling between patient homes
Strong understanding of geographic routing, travel optimization, and territory management
Experience managing frequent schedule changes and same-day adjustments
Excellent organizational, communication, and multitasking skills
High attention to detail and ability to work independently
Comfortable working in a fast-paced, rapidly growing startup environment
Proficiency with EMRs, scheduling software, Microsoft Office, and Google Workspace
