Clinical Director – Physician Leader of Care in Home Hospital Program

About the role
Key responsibilities & impact- Join our team as the Clinical Director, Care in the Home, a senior physician leader responsible for shaping and scaling innovative home and community-based care models.
- Provide strategic, clinical, and operational leadership across programs including Hospital at Home, Advanced Care at Home, ED/Urgent Care at Home, and Care After Discharge.
- Maintain clinical practice through direct patient care (approximately 0.3 FTE) with prorated participation in weekend and holiday coverage.
- Drive development of care pathways, readmission reduction initiatives, and post-acute care partnerships.
- Champion virtual care, remote patient monitoring, and innovative technologies that extend care beyond traditional settings.
- Mentor physician and advanced practice clinician leaders while fostering a culture of excellence, accountability, and innovation.
- Directly supervise three clinician leaders who collectively oversee 12 physicians and APCs.
Requirements
What you’ll need- Board-certified physician with an unrestricted Delaware medical license and DEA.
- Minimum 10 years of clinical experience in hospital medicine.
- At least 3 years of leadership experience overseeing clinical programs or care delivery operations.
- Experience with telehealth, virtual care, and value-based care models required.
- Advanced management degree or certification (MBA, MPH, MHA, MPA, ACPE) preferred.
